The LifeScape Patient's Responsibility

As medicine becomes more complex, patient care requires a team effort. The more you know about your own health and take an active role in researching your health issues, the better your care will be. In order to get the most from your medical care, we ask that our patients take responsibility for:

Knowing their personal health history and keeping a written medical record that includes:

Current medical problems;

Past medical history including hospitalizations;

Surgical procedures and invasive tests;

Vaccines and immunizations;
-
All current medications including vitamins, herbals, and other nutritional supplements;

Medication allergies or intolerances;

Preventive health schedule including date of last pap smear; and mammogram, prostate or colon cancer screening.

Knowing their family health history for, ideally, two generations in each direction (i.e., grandparents, parents, siblings, children, and grandchildren).

Being informed about their health issues, asking questions, and not being afraid to challenge us.

Using their health provider to guide and filter information obtained from other sources.

Being on time for visits. Arriving late not only disrupts our schedule, but also the schedule of each patient who follows you. Please be courteous and arrive on time.

Notifying us promptly of a cancellation. Failing to appear for an appointment not only disrupts our schedule, but also harms a patient whom might otherwise have been seen during that period. To encourage prompt notification, LifeScape charges $75 for any missed appointment or appointment not cancelled within six hours of the appointment start time for any reason.

Assisting us in conducting research, advocacy, and improving our services. Patient participation and feedback are essential to quality healthcare both at LifeScape and throughout the community.
