Knowing their personal health history and keeping a written medical record
that includes: |

and immunizations; |

All current medications including vitamins, herbals, and other nutritional supplements; |

Medication allergies or intolerances; |

Preventive health schedule including date of last pap smear; and mammogram, prostate or colon
cancer screening.

Knowing their family health history for, ideally, two generations in each
direction (i.e., grandparents, parents, siblings, children, and grandchildren).

Being informed about their health issues, asking questions, and not being
afraid to challenge us.

Using their health provider to guide and filter information obtained from
other sources.

Being on time for visits. Arriving late not only disrupts our schedule,
but also the schedule of each patient who follows you. Please be courteous
and arrive on time.

Notifying us promptly of a cancellation. Failing to appear for an
appointment not only disrupts our schedule, but also harms a patient whom
might otherwise have been seen during that period. To encourage prompt
notification, LifeScape charges $75 for any missed appointment or appointment not cancelled within six hours of the appointment start time for any reason.
