Call In For Your Results
 
LifeScape's "Call In For Results" service enables patients to access recorded laboratory and test results, doctor's instructions, and other pertinent information. If you have lab work or tests performed at LifeScape, we may notify you that there is a message ready for your retrieval. To retrieve your message:
Step 1:
Using a touchtone telephone, call 480-423-7124.
 
Step 2:
To listen to the prompts in English, press “1”, in Spanish, press “2”.
 
Step 3:
Enter your nine-digit patient identification number by dialing “0” followed by your month, day, and year of birth in the format MMDDYYYY.
 
For example:
 
September  4, 1980       " 0 09041980 "
November 15, 1955       " 0 11151955 "
 
Step 4:
Record your name at the prompt then press "1" on your telephone keypad.
 
Step 5:
At the end of the recorded message press "1" to repeat the message, "2" to delete the message, or "3" to save the message.
 
You may save a message for up to two days from the time you first listen to it. At the end of the two days, the message will be deleted from our system.
 
Please be sure to listen to your message in its entirety. Notify us of any changes in your home phone number to ensure uninterrupted communications.
